Just download the forms, print them at home, fill out the best you can and bring to your first appointment. Remember: you still need to bring your insurance card for us to scan. Thanks!

How do I get my records to NVOG when I've been seen at another facility?
You can contact the facility where you were previously receiving your care, fill out & sign a release of info. and you can have them fax to our office at 701-738-2005 and we'll take care of it from there.

I wonder if I'll have to pay a copay?
Copays are a specified dollar amount payable for certain covered services that is your responsibility to the provider. Every contract is different for every patient. Its a good idea to call your insurance company to see if you are required to pay at time of your visit. We accept debit/credit cards for your convenience.

What does an independent clinic mean?
Northern Valley Obstetrics & Gynecology is an indepedent private-owned OBGYN office separate from other clinics here in the Aurora Clinic building. We all share the same space but we all work independently of each though communicating to each other to make your clinic visits seamless.
